Program Description

Provide support in the community to individuals 16 years plus who experience a serious and persistent mental illness, however a diagnosis is not required.. This may include individual service planning, symptom management, supportive counselling, life skills training, medication support, education, and advocacy. Model of service is Brief Case Management.
