Case Management/Supportive Counselling & Services - Mental Health

Program Description

This service provides specialized assessment, support and case management services to individuals who have a serious mental illness (and their families) who are 60 or older. Health teaching, counselling, and support is provided by a multidisciplinary team consisting of psychiatric nurses, social workers, and mental health case workers with specific training in psycho-geriatric issues. Specialized group for seniors with depression provided. Model of service is recovery, best practice related to psycho-geriatrics.

Access Information

Referrals and program inquiries may be made to the program intake worker Monday to Friday 0830-1600. Self-referral, organization referrals, family or physician referrals are welcome. Psychiatric assessments are available through OTN with a doctor's referral.
