Program Description

Provide consultation for adults age 16 and over living with mental illness and addictions, or vulnerable seniors. The psychiatrist provides consultation through videoconferencing equipment in a community close to the client's home. The primary care provider provides ongoing monitoring of the treatment recommendations.

Access Information

Physician referral is required. To make a referral, call anytime to leave a message or receive the referral form, or fax the completed, signed referral form along with any supporting documentation/lab results.
