Case Management/Supportive Counselling & Services - Mental Health

Program Description

Case management services are offered to individuals experiencing severe difficulties due to persistent mental illness. Outreach services encourage independent living with a supportive counselling approach. Clients of the service are offered a wide range of services including advocacy, monitoring of symptoms, education, empowerment, recreational programming, life skills, community re-integrations, financial management and education, housing support and referrals and support in accessing other community services. We offer focus group sessions on various topics. Treatment plans are individually tailored to support a community based recovery model.

Access Information

Referrals are received from various sources. Screening appointments are available weekdays and contact will be made within 48 hours of the initial referral. All referrals are reviewed during a common referral process for Case Management. Clients may access both mental health and substance abuse services through the same intake process. Appointments may be made by phone or in person. Referrals will be made to the most appropriate services within the system.
