Case Management - Mental Health

Program Description

Assists and support individuals who have a major mental illness and have a criminal history. Individuals are supervised and supported by a forensic case manager who facilitates psychiatric treatment, referrals to community services, housing and financial assistance, family intervention, crisis management and mental health monitoring.

Clients referred must have a current criminal charge or conviction, and a major mental illness diagnosis.

Access Information

Referrals are made by physicians, probation officers, or community agencies to the program via fax.
