Program Description

Assertive Community Treatment (ACT) is a community-based model of care for individuals who have severe and persistent mental illness. It reaches out to people assertively where and when they need help directly in their home and in the community. ACT brings a team of healthcare professionals together to provide highly integrated and individualized services for individuals.

Referral from psychiatrist required. Centralized Access through the Sarnia Lambton Mental Health Access Committee.
