Assertive Community Treatment Teams

Program Description

The ACT Team mental health professionals take mental health service to the client's home, community or hospital. The goal is to promote recovery, improve quality of life and help clients to achieve their goals through individualized treatment, rehabilitation and support programs. Model of service is active treatment and psychosocial rehabilitation.

Access Information

ACT is for people with complex, long term and serious psychiatric illness involving multiple hospitalizations. Referrals can be made by the individuals, the family or current service provider. Applications for all Ottawa-area ACT Teams are reviewed by a central intake team on a monthly basis. ACTT central intake process can take up to a year with priority given to clients currently hospitalized more than 90 days.
