Program Description

The Assertive Community Treatment (ACT) Step-down team is an interdisciplinary team of mental health professionals who work in partnership with clients who are living with serious and persistent illness. The ACT Step-down program brings mental health services to the client's home, community or hospital. Model of service is case management with a recovery focus.

Access Information

Services are offered to previous clients of ACT who require a less intensive level of support. The intake process takes two to three weeks. The referrals come from the 5 Ottawa ACT Teams.
