Program Description

Case managers provide individualized, flexible and client-directed support to individuals living with severe and persistent mental illness. The focus is on client strengths, interests and abilities. The service is provided in the client's environment, and is portable and flexible, following the client when he/she changes housing. Service is mobile but there is also use of the phone. The model of service is recovery-oriented with a psychosocial rehabilitation focus. Recreology and Occupational Therapy services are available for clients in the program.

Access Information

People living in three designated Ottawa Community Housing buildings (415 Maclaren; 1400 Lepage; 2651 Regina) are referred to this service by Ottawa Community Housing tenant workers and Ottawa Salus community developers. To be referred to this program people must be at risk of losing their housing due to untreated mental health and/or addictions. The goal of this service is homelessness prevention.
