Program Description

Case managers provide individualized, flexible and client-directed support to individuals living with severe and persistent mental illness. The focus is on client strengths, interests and abilities. The service is provided in the client's environment, and is portable and flexible, following the client when he/she changes housing. Service is mobile but there is also use of the phone. The model of service is recovery oriented with a psychosocial rehabilitation focus. The provision of in-house recreology and occupational therapy services. Available for clients in the program.

Access Information

There is a coordinated access function, provided through Mental Health Community Support Services (MHCSS) based at the Canadian Mental Health Association Ottawa Branch. MHCSS maintains the waiting list for intensive case management in Ottawa, referring applicants to several local agencies, including Salus.
