Case Management/Supportive Counselling & Services - Mental Health

Program Description

Youth Services support youth at risk and transitional aged youth, and include: organization outreach - initiate connections between youth and adult services to assist with service response and enhance planning activities in the transition from youth mental health services to adult mental health services; youth outreach - seek out and identify youth at risk via traditional and non-traditional approaches; early intervention - for those youth who are not ready to transition to the adult system, initial intervention will be provided to reconnect them to youth services, education and their families; intervention support - for those youth who are at risk and wish to transition to the adult system, a Youth Outreach Coordinator will provide case management support to assist with a successful transition and integrate them into the community. Drop-in groups are also available.

Access Information

Referrals are processed by one-Link, a team that reviews referrals in our area to make sure that people who need support receive the right services for them. The fax number for one-Link is included on these referral forms. After a referral is made, individuals will be contacted by a one-Link Service Coordinator to talk about the referral in more detail.
