Program Description

Housing First Intensive Case Management (HFICM) provides support to individuals in Toronto who are experiencing homelessness, and are living with mental health issues. It is our belief that all people deserve housing, and that adequate housing is a precondition for recovery. We collaborate with clients in locating housing, and provide them with the support they need to maintain their housing and achieve their goals. Through regular meetings with a case manager (in our offices or in the community). HFICM provides: help to obtain adequate housing; support with making social and community connections that will assist in recovery; supportive counseling, physical assessment, and medication management as needed.

Organization's Current Reported COVID-19 Response

CMHA Toronto main offices and 24/7 residential programs remain open with social distancing in effect. All drop-in programs and groups such as Routes, Pathways and Women’s Groups are suspended unless you have been notified if they are taking place online or over the phone. Our staff are providing peer support for members over the phone. Non-essential in-person appointments will be limited or rescheduled.
