Case Management/Supportive Counselling & Services - Mental Health

Program Description

ReLinC Services is a community-based case management service for individuals aged 16 and over, with a history of serious and persistent mental illness. Case managers work in partnership with clients to develop the skills and supports they need to live successfully in the community. They meet with clients either in their own home or in the community. Model of service is case management. Average length of stay is 2-3 years.

Access Information

Clients are referred by health care professionals, family and community organizations. Self-referrals are also accepted. Referrals are through one-Link.
