Case Management - Mental Health

Program Description

Interim support and case management provided to people identified as in urgent need in the area of mental health and addictions. Services will include care planning and case management; treatment planning; short term counselling and support; crisis intervention and prevention; coordination of services; and referral for First Nation, Inuit, and Métis individuals identified as urgent and/or in transition between services.

Access Information

Referrals accepted from all sources. Call or complete and submit the online intake form.

Eligibility: Individuals must have registered status, Métis card, or documented affiliation with a First Nation community.
