Program Description

This program helps individuals to identify meaningful goals and effective ways for each person to reach those goals. Team members work alongside individuals, enabling individuals to exercise their rights and to enjoy a meaningful life in their community while striving to achieve their full potential. Team members work to promote health, housing, purpose, and community. The program uses a holistic, strengths-based approach to support individuals towards living a full, independent, and satisfying life. This is a time limited services that is focused on supporting individuals to live as independently as possible.

Access Information

Referrals accepted from all sources. Referrals can be made by calling the Central Intake Team. For individuals living with a serious mental illness and/or substance use disorder, and have a willingness and a commitment to work on personal recovery goals. Referral Form is available on the CMHA Grey Bruce website.
