Program Description

The Dual Diagnosis Specialist works with individuals, their families and other service providers to develop crisis prevention plans, facilitate community integration, provide case management support, pre-consultation information gathering for psychiatry, and coordinate developmental and mental health services.

If you need to speak with them sooner, please leave them a message at their extension. They will endeavour to get back to you within 24 hours. We are not offering walk-in appointments at this time. Access Information

Self-referrals or by family, physicians, hospitals or community organizations are received through the Sarnia Lambton Mental Health Access Committee. Contact CMHA for assistance in making referral. Access committee meets weekly to review referrals. Services are for individuals that have a severe and persistent mental illness causing disability.
