Assertive Community Treatment Teams (ACTT)

Program Description

For individuals who have complex bio-psychosocial needs due to severe and persistent mental illness.

Intensive treatment, rehabilitation and support include; assertive outreach, 24-hour crisis service provided in conjunction with Sault Area Hospital Crisis Services, supportive counselling, medication monitoring, skills building and advocacy.

Access Information

Self referrals are accepted. Must meet ACT criteria. High prior hospitalization, emergency, and/or community involvement.

Priority given to transfers from other ACT teams, long-term psychiatric units and schedule one facilities.
