Assertive Community Treatment (ACT) is client-centered, recovery-oriented mental health service delivery model that facilitates community living, psychosocial rehabilitation, and recovery for persons who have the most serious mental illnesses, have severe symptoms and impairments, and have not benefited from traditional outpatient programs. ACT Timmins serves clients throughout the district of Timmins. Important characteristics of the program are: ACT serves clients with serious mental illness that are complex and have devastating effects on functioning, and who, because of the limitations of traditional mental health services, may have gone without appropriate services. Consequently, the client group has often been unfairly thought to resist or avoid involvement in treatment. ACT services are delivered by a group of multidisciplinary mental health staff who work as a team to provide the majority of the treatment, rehabilitation, and support services clients need to achieve their goals. The team is directed by a team leader and a psychiatrist and works in shifts to cover 24 hours per day, seven days a week to provide intensive services (multiple contacts may be as frequent as two to three times per day, and are based on client need and a mutually agreed upon plan between the client and ACT. The services are individually tailored with each client and address the preferences and identified goals of each client. The approach with each client emphasizes relationship building and active involvement in assisting individuals with serious mental illness to make improvements in functioning, to better manage symptoms, to achieve individual goals, and to maintain optimism. The ACT team delivers services in community locations to enable each client to find and live in their own residence and find and maintain work in community jobs rather than expecting the client to come to the program. Seventy-five percent or more of the services are provided outside of the program offices in locations that are comfortable and convenient for clients. ACT services are delivered in an ongoing rather than a time-limited framework to aid the process of recovery and ensure continuity of a caregiver. The model of service is assertive community treatment.

Note that the 12-Step program is presented in a manner that respects the individual beliefs, values, and choices of abstinence. The TrueNorth opioid support program is dedicated to helping people who are experiencing problems related to opioids.The term "opioid" refers to a group of analgesics (painkillers) that includes medications such as codeine, heroin, morphine, fentanyl, oxycodone, hydromorphone etc. These drugs can be extremely powerful pain medications, but they can also be very dangerous and highly addictive. In Ontario, twice as many are dying from opioid as are dying from HIV.
There are a lot of misconceptions out there in terms of opioids. One of the biggest myths is that only "recreational" and/or "street" users of opioids are at risk of negative consequences from their opioid use. This is not the case at all. In fact, more than half the patients we see were started on opioids for pain and have never bought an opioid on the street. The treatment of opioid-related problems is often very complex and challenging. For this reason, our team uses a multidisciplinary team environment that includes counselors, psychiatrist and addiction medicine specialists. We also take pride in using state-of-the-art technology and incorporating the latest advances in health care and opioid research.As with all of our TrueNorth programs - Our goal is to provide all of our clients with first-rate care in an environment that is safe, respectful and confidential. All of our services are covered by OHIP.

Specialized Diagnostic Assessment measures the severity of the eating disorder diagnosis. It consists of a structured interview based on the DSM-V criteria for eating disorders, followed by the administration of eight psychometric tests that measure related psychopathology (i.e., depression, anxiety, alexithymia, obsessive-compulsive disorder, attachment style, and family functioning). The Specialized Diagnostic Assessment is administered before and after each treatment. The Specialized Diagnostic Assessment is the first step and required prior to admission to treatment.

This service consists of an interdisciplinary team to assess and provide a broad range of clinical and support services to individuals (and families) experiencing a first/early episode of psychosis. This program is a multi-disciplinary intensive assessment and treatment program for adolescents and young adults (ages 14-34) who have experienced their first episode of psychosis during the previous year. This is an early intervention program with a focus on early recognition and comprehensive services during the early onset of the disease in order to avoid a prolonged course of illness and reintegration back into social, emotional, activity and vocational functioning. The EPI team focuses on client recovery while working collaboratively with the client's family, school and community. Family support and education is offered to all clients. Average program length is three to five years.

Ontario Shores Centre for Mental Health Sciences (Ontario Shores) has a 12-bed assessment, stabilization and treatment unit for patients who require specialized care in a secure setting. Adolescent Services has an interprofessional team of healthcare professionals that includes psychiatrists, nurses, child and youth workers, social workers, psychologists, occupational therapists and other allied health professionals. Together, they provide ongoing assessment, treatment and life skills training for youth and adolescents. Eligible individuals are 12 to 17 years of age with a mental health issue and living in the province of Ontario. They must have had a previous psychiatric assessment and have received services and/or treatment in their home community that has not led to sustained recovery.

The Eating Disorders Unit is a 12 bed unit for children and adolescents struggling with an eating disorder. It provides an interprofessional model of care with a focus on recovery and rehabilitation. There is a pre-admission process, post-discharge support and planning for transition home. The unit includes individual patient bedrooms, kitchen and dining room and lounge area. There are also group rooms and spaces for patient, family and group therapy. Parent and family involvement is a key component of the clinical programming. During their time at Ontario Shores, patients will also have access to teachers who can assist with their education needs. Programming includes: Intensive individual therapy; group and family therapy; Psychoeducation; Meal support, nutrition and eating rehabilitation; Psychopharmacologic treatment; Schooling; Treatment for co-morbid disorders. Modalities used include; CBT, DBT, EFT and FBT. Inclusion criteria: The client must be between 12 years of age and 17 years of age and the referral must be submitted at least three months prior to their 18th birthday; All treatment options for eating disorders available in the client's community have been unable to assist the patient reach recovery; The client is unlikely to benefit to a less intensive treatment other than residential care; Serious eating disorder symptoms continue after less intensive treatment in the community have been undertaken. i.e. day treatment and outpatients if available in community; Referral source is able to commit to recommended follow-up after the teen has completed the Ontario Shores Eating Disorder program; Referral source is able to accept or arrange medical transfer should the patient become medically unstable; Patient be medically stable, orthostatic vital signs, blood work, and EKG must be stable.

Schizophrenia Outpatient Clinic (SOC) is an outpatient program dedicated to providing a collaborative, person-centered approach to the assessment, treatment and rehabilitation of individuals with schizophrenia and related psychotic disorders with a focus on self-management and recovery. Schizophrenia Outpatient Clinic (SOC) is also open to consultation, appointments specific to diagnostic clarification and medication reviews. Client registered to receive follow-up at Schizophrenia Outpatient Clinic (SOC) will be provided with a psychiatrist and one healthcare provider referred to as the "Most Responsible Clinician." The MRC is the person the client sees the most and goes to when support is needed. The MRC could be a nurse, social worker, vocational counsellor, recreation therapist or occupational therapist. The MRC will help clients develop personal goals for optimal health and meaningful participation in life. Also, a client may access a variety of mental health providers for individually tailored assessment, consultation and support including psychosocial treatments. Group treatment is provided through a variety of therapeutic groups to address personal growth, self-management and wellness.

The Anxiety Treatment and Research Clinic (ATRC) is an outpatient program dedicated to providing clinical service, education, and research in the area of anxiety disorders. Comprehensive assessment services are provided for a full range of anxiety-based problems. In addition, the ATRC has treatment programs for individuals aged 17-75 suffering from obsessive/compulsive disorder (OCD), panic disorder, social phobia, generalized anxiety disorder and post-traumatic stress disorder. These include group and individual interventions, typically lasting 10 to 15 sessions. In some cases, brief treatments are also available for individuals suffering from health anxiety and specific phobias. Treatment programs include cognitive behavioural therapy, medications, or combinations of these approaches. Individuals who receive treatment in one of the ATRC's standard programs are later welcome to attend a monthly booster group designed to reinforce the skills learned during the initial course of treatment. Model of service is evidence-based psychological treatments (e.g., cognitive behavioural therapy) and pharmacotherapy offered on a short-term basis. Assessments are normally conducted over one to two sessions. There may be a wait of a few months between the initial assessment and the start of treatment. The Anxiety Treatment and Research Clinic also provides on a non-urgent basis, a consultation service for children and adolescents between the ages of 6 to 17 years with a probable diagnosis of Obsessive Compulsive Disorder (OCD) only. This service provides diagnostic assessment and clarification, treatment recommendations and medication consultation.

At the Mood Disorders Inpatient Program, our mission is to offer the best possible treatments, those proven by research, for people who have a refractory mood disorder and require hospitalization. The inpatient program is a 24-bed program that provides clinical care and treatment and prepares patients for their return to the community. The inpatient program optimizes a variety of proven treatment strategies with aim of controlling the symptoms of a mood disorder, as well as supporting the recovery and rehabilitation of the person. Along with pharmacotherapy, several group therapies are offered including stress management, cognitive behaviours therapy (CBT) and activities of daily living. Our services include: rapid psychiatric assessment, treatment based on individual concerns/goals including education about psychosis, assistance with work, school, social and leisure activities, specialized support to family members and friends, addictions counselling, and introduction to other community programs and planning for management of illness after discharge from the program. The staff available to support clients include: family educators, nurses, occupational therapists, psychiatrists, social workers, addiction clinicians and peer support/recovery mentors. Model of service is recovery-oriented, emphasizing: client choice, flexibility in services, individual supports, and the importance of families, significant others and communities. This approach also considers the impact of factors such as poverty, poor housing, unemployment and stigma on people with mental illnesses. The TrueNorth opioid support program is dedicated to helping people who are experiencing problems related to opioids.The term "opioid" refers to a group of analgesics (painkillers) that includes medications such as codeine, heroin, morphine, fentanyl, oxycodone, hydromorphone etc. These drugs can be extremely powerful pain medications, but they can also be very dangerous and highly addictive. In Ontario, twice as many are dying from opioid as are dying from HIV.
There are a lot of misconceptions out there in terms of opioids. One of the biggest myths is that only "recreational" and/or "street" users of opioids are at risk of negative consequences from their opioid use. This is not the case at all. In fact, more than half the patients we see were started on opioids for pain and have never bought an opioid on the street. The treatment of opioid-related problems is often very complex and challenging. For this reason, our team uses a multidisciplinary team environment that includes counselors, psychiatrist and addiction medicine specialists. We also take pride in using state-of-the-art technology and incorporating the latest advances in health care and opioid research.As with all of our TrueNorth programs - Our goal is to provide all of our clients with first-rate care in an environment that is safe, respectful and confidential. All of our services are covered by OHIP.

Guided by the principles and values of psychosocial rehabilitation, Supportive Housing provides community living for people who have a serious mental illness and require treatment, rehabilitation and support. St. Joseph's Care Group partners with a non-profit community housing corporation to provide affordable housing accommodations of which clients are rent paying tenants. High Support housing is geared for people with a serious mental illness who consistently have difficulty: maintaining personal hygiene; meeting nutritional needs; maintaining a household; managing finances; accessing transportation independently; communicating; managing medication and personal health; participating in leisure/social/vocational activities. Three 6-bedroom furnished homes and one-12 bedroom furnished home provide a home for a total of 30 clients requiring 24 hour on-site clinical support, rehabilitation and treatment. Three houses are on the north side of the city, one house is on the south side of the city. Clients work collaboratively with nursing staff and Community Support Assistants to achieve their optimum level of independent living and to attain their personal goals. Tenants are supported to share the responsibility of maintaining the common areas in the home and to plan and prepare meals. Clients are supported to manage their finances and their health care needs and to explore and participate in leisure, socialization and work activities. Clients able to develop skills to live in a less supportive environment are encouraged to do so.

This program is offered, as a part of STC's residential program, to women who are pregnant and pursuing residential treatment. The program is five to six month residential treatment program for chronic and acute substance abuse issues that is based on a progressive therapeutic community model of treatment. The program is holistic with a multi-disciplinary approach (biopsychosocial, cognitive behavioural, stages of change, psychodrama, experiential therapy, and psycho-educational). It is a highly structured program that is group-based with self-help and mutual-support components. Beds are continuous intake; there is a 1-bed capacity which is shared with the Residential Program - Women.

There is 24-hour support staff. Residents must participate and abide by community guidelines. During the first week of orientation there is no off-site communication permitted; these privileges increase as residents’ progress through the program. Case Management is available to coordinate OBGYN appointments and there is additional parenting programming. Support coordination is provided for residents on Opioid Substitution Therapy (OST). Residents participate in a weekly OST clinic with a physician via Ontario Telemedicine Network (OTN) and submit urine screening.

Bi-weekly support groups and individual sessions are offered. The Residential Support Counsellor works with the Admissions Coordinator to ensure a smooth transition of residents' opioid substitution therapy services (physician, urine screening, pharmacy etc.) from the community into the residential program. Discharge planning is supported for a smooth transition of services from treatment back into the community through referral and support. Smoking cessation is available through collaboration with the Guelph Community Health Centre and the Smoking Treatment for Ontario Patients (STOP) program. Friends and family are invited to attend the Friends and Family Support Group, held on the third Saturday of every month. Friends and family can attend the support group while their loved one is in treatment and for two additional years provided that their loved one completes a minimum of three months of the residential program. The Supportive Addiction and Mental Health Housing (SAMH) program in Guelph-Wellington is provided by Stonehenge Therapeutic Community in partnership with the Canadian Mental Health Association Waterloo Wellington Branch. The program is designed to support persons with problematic substance use or concurrent mental health and substance use issues who are homeless, at risk of becoming homeless, or are considered inadequately housed.

Applicants must also demonstrate financial need. Participants are accepted into the Transitional Program for a period of 6 months, and can submit a written request to extend their time in the program for up to a total of 364 days. Extensions will be granted based on the individual’s identified recovery goals and their demonstrated active participation in the program. Services are delivered from an abstinence-based perspective. Transitional housing is available to those who meet the above-noted criteria, who have recently participated in residential or day addictions treatment and are seeking to stabilize an abstinent recovery in the community.

The Senior's Mental Health Program is a regional consultation program available through the North Bay Regional Health Centre - College Drive campus. Services are offered throughout the district of Timiskaming by referral from a family doctor or health care provider. An assessment is completed by a mental health nurse followed by a psychiatric consultation with a psychiatrist who then provides the family physician or other health care provider with diagnosis and treatment recommendations. Referrals are generally seniors over the age of 65 or younger persons who have mental health problems or illnesses. Services are also provided to people in long-term care facilities who have behavioural problems related to dementia or to mental illness. This is not a crisis service and clients must be medically stable. Once the assessment and psychiatric consultation have been completed, the treatment recommendations will be followed through by the referring health care provider. Referrals to other supports will also be made. Limited follow-up for people with age-related mental illness.

The goal of this initiative is to increase the health and social outcomes of people with problematic substance use who are frequent users of the addiction treatment system and emergency services by providing stable housing and appropriate support services. We use a harm reduction approach to service delivery. All tenants will be supplied with a one-bedroom apartment and the support of an Addictions Counsellor. Services provided include supportive counselling, life skills teaching, crisis prevention and intervention, recreational activities, group work, and support from our psychiatrist, nurse and trustee. We also offer mindfulness training and acudetox auricular acupuncture on-site.

Pieces To Pathways is a peer-led substance use support program for LGBTTQQ2SIA youth (age 16-29) in Toronto. We offer three drop-ins, case management, and group counseling. Our drop ins are an opportunity for LGBTTQQ2SIA youth to connect with others, access support, play games, attend workshops, watch movies, and go on group outings. We offer an abstinence drop in on Mondays, a drop in for racialized youth on Tuesdays, and a harm reduction drop in on Thursdays. Tokens and meals are provided, with meat, vegan, and gluten-free options available. Case management support includes identifying and pursuing a range of goals such as those around substance use, housing, employment, and mental health. We provide general counseling, one on one sessions in office or in the community, and phone and text message support. Our group counseling sessions utilize a motivational interviewing framework and teach DBT skills to help participants with emotional regulation and triggers. We additionally have a partnership with the Academic Family Health Team at Saint Michael's Hospital, allowing participants expedited access to LGBTTQQ2SIA-positive primary care services.

Cota Health's Bailey House supportive housing program provides rent subsidized accommodation and support for 10 men living with mental health challenges and co-occurring diabetes or pre-diabetic conditions. Each tenant has his own bedroom and shares a bathroom and communal kitchen with other tenants. Cota Health provides on-site support during the day, seven days per week. Overnight security is on-site seven nights per week. The staff work collaboratively with tenants, their families, the landlord and other clinicians, as appropriate, to assist tenants to: develop an individual safety and housing support plan; maintain their housing stability and a healthy housing environment; acquire and/or maintain the skills necessary for daily living; participate in health promotion and community development activities such as tenant meetings, structured educational events and social activities; resolve situational problems/disputes; link to other desired community services/supports, as needed; achieve/maintain optimum health and wellness; and pursue personal recovery. Cota Health provides one meal per day on Saturday, Sunday and Monday. Tenants are responsible for their own meals outside of these scheduled times.

We offer medical, psychological, and complementary care for people with addictions. As part of our Comprehensive Care Program, we offer Opioid Agonist Therapy, which involves prescribing medications to help people stop using opiates (such as heroin, fentanyl, morphine, dilaudid, codeine, oxycodone, etc.). We also offer individualized treatment for alcohol and other drugs using both medication and behaviour therapy. Non-medication treatment options including psychotherapy, cognitive behavioural therapy (CBT), therapeutic art, acupuncture detox, mindfulness meditation, and spiritual guidance are offered to help address the issues underlying addiction. Dr. Bienenstock can also provide on-demand primary care services to his patients during their recovery journey. Additional support services include access to shelter or housing options, access to food banks, a link to employment agencies, referral to residential treatment, and access to 12-Step recovery groups.

The Meeting Place Adult Drop-in provides services and programs for a diverse group of people, including 40% Indigenous men and women, primarily members of our community who are homeless, living in poverty and socially isolated. We offer a safe environment where people can meet and socialize in a non-stigmatizing atmosphere and participate in a variety of activities and become part of a community where they feel accepted and valued. We provide a range of services where people's basic health and hygiene needs can be met by accessing our laundry, showers, storage services, and kitchen facilities. Staff works closely with members to help them gain access to needed external resources and assist with information and referrals for housing, health, employment, legal assistance, addiction counselling and social assistance. Members and staff meet to talk about pertinent issues, make plans for recreational activities, inform one another about events in the drop-in and community, and lend support to one another in various ways. Through a community development approach, the drop-in works to improve people's lives above and beyond the basics and re-connect with the broader community. Member-driven activities include the community kitchen, culturally specific programming for Indigenous participants, clinics specific to income, legal, health and immigration supports, arts, crafts, odd jobs and courier programs. Members can become more involved with their community through social networks, advocacy and community economic development initiatives. Model of service is adult drop-in that emphasizes community development/community empowerment, case management, individual peer support, leadership development and community economic development programming.

Street Haven provides safe, affordable, permanent housing and supportive housing services to thirty-two single women at three locations in Toronto. Community outreach and aftercare services are also offered. Street Haven counsellors assist each woman in actively participating in setting and achieving personal goals towards independent living. Counsellors help women maintain healthy, productive lifestyles. Assessment, life skills teaching, community referrals, advocacy and monitoring may be among the components of an individualized case management plan. Twenty-four hour crisis support is available. The Residential Tenancies Act applies to all our housing units. Joubert House is a six-bed residence for women who have histories of serious mental illness and homelessness. Residents must make a commitment to accept support counselling, to work towards further independence, to attend weekly house meetings, to participate in group activities and to abstain from drugs and alcohol. In addition, we have twenty-six self-contained apartments for women with histories of addictions, mental illness or concurrent disorders. Six months of sobriety is required to qualify for the abstinence-based housing. Residents must also be willing to accept supportive counselling, to work towards further independence and to attend residents' meetings.

The TrueNorth opioid support program is dedicated to helping people who are experiencing problems related to opioids.The term "opioid" refers to a group of analgesics (painkillers) that includes medications such as codeine, heroin, morphine, fentanyl, oxycodone, hydromorphone etc. These drugs can be extremely powerful pain medications, but they can also be very dangerous and highly addictive. In Ontario, twice as many are dying from opioid as are dying from HIV.
There are a lot of misconceptions out there in terms of opioids. One of the biggest myths is that only "recreational" and/or "street" users of opioids are at risk of negative consequences from their opioid use. This is not the case at all. In fact, more than half the patients we see were started on opioids for pain and have never bought an opioid on the street. The treatment of opioid-related problems is often very complex and challenging. For this reason, our team uses a multidisciplinary team environment that includes counselors, psychiatrist and addiction medicine specialists. We also take pride in using state-of-the-art technology and incorporating the latest advances in health care and opioid research.As with all of our TrueNorth programs - Our goal is to provide all of our clients with first-rate care in an environment that is safe, respectful and confidential. All of our services are covered by OHIP.

The Assertive Community Treatment (ACT) teams provide the treatment, rehabilitation, and support services that assist people with severe mental illness in their recovery and their desire to live in the community. The teams are multidisciplinary each having a social worker, nurses, a vocational specialist, an occupational therapist, a psychiatrist, a peer support specialist, and an addictions specialist. ACT teams are able to provide services in the community, including people's homes. Support can be very intensive and a 24-hour on-call system is available to clients. Canadian Mental Health Association (CMHA) Toronto has three ACT teams. West Metro Act Team and East Metro Act Team are designated forensic (which means that they work with consumers who have been involved with the legal system and have been referred by the Ontario Review Board). Model of service is an ACT model. Average length of stay is three years.
