Durham Mental Health Services provides a range of high, medium, and low support housing options, enabling clients to progress through the stages of housing as greater independence is sought and achieved. Each resident works with a primary counsellor to set their own individualized goals. McKay House is double-staffed 24 hours a day and provides the highest level of support. Residents receive support in areas including activities of daily living, life skills, medication management, accessing community services, socialization, and crisis management. As residents develop their skills, supports, and routines, they progress to medium-support (single staffed 24 hours a day) and low-support (40 hours staff support weekly) housing.

The team provides assessments for individuals experiencing acute emotional and psychological difficulties. Clients are first assessed by the emergency department and then referred immediately to the Crisis Intervention Team, including a psychiatrist. Individuals often arrive with the police, a community mental health worker and/or a family member. Most clients are discharged with referrals and information regarding community supports, while others (one quarter) may be admitted to the Inpatient Mental Health Program. Involuntary and voluntary clients are both appropriate. Model of service is medical. Average length is typically a single session (one to two hours).

The Justice and Mental Health Program is designed to assist and support individuals who are suffering a major mental illness and have a criminal history. Community and professional education are also part of the program's mandate. The release from custody/discharge planning part of this program will provide outreach for individuals with mental illness who are being released from custody into the community. Model of service is psychosocial rehabilitation. Average length of stay is one to two years. Average length of stay for release from custody is 4 - 6 months.

The Warm Line is NOT a replacement for the Crisis and Invention Unit, the Police, the Hospital or any other Emergency Need. It is an ALTERNATIVE to calling the Police, Hospital or Crisis & Intervention when a Survivor needs to know someone is available and willing to listen just like a friend and NOT an emergency situation. Volunteers are required to contact the local authorities when they feel an emergency situation is occurring and will provide as much information as possible to ensure the safety of the caller in the case of an emergency or if a Volunteer cannot assist they will also call the on-call Supervisor (staff). You do not have to provide your provide your full name, but a name to call you when talking is awesome. What to talk about.... Everyday Life Concerns & Issues for any reason, including but not limited to mental health, bill issues, friend problems, where to find something you need or anything that will help you with everyday life.

This group offers support to people who care for someone with a mental illness. Groups are offered throughout the year and meet once per week over a 10-week period. Groups are offered in multiple locations. Through participation in the group, participants will increase their understanding of mental illnesses, relevant medications and their side effects enabling them to make more informed decisions; reduce the stress they face through increased access to information, services, and a support network; meet other people in like situations for coaching, mentoring, brainstorming, problem resolution, and sharing; learn about stigma and its impact on recovery; and, develop a plan of action for self-care.

The case worker will focus on the development of a natural support network with the client and will work with the client towards a discharge plan from the day of the intake. The CTYS ERSP is a four- to six-month outreach-based program that supports youth who are awaiting or have been released on bail. ERSP workers offer support to youth as well as their parents. Intake will be an introduction and orientation to the organization. Assessment will gather information as part of service planning. Service coordination and referrals to link consumers with appropriate internal organization services/supports and external community services/supports. Maintenance support by monitoring of clients holistic well-being with supportive listening and awareness teaching of coping skills and interventions. Mental Health Rights Coalition (MHRC) is a consumer/survivor initiative funded by the Ministry of Health through LHIN 4. The MHRC's mission is to encourage, enable and empower the voice and participation of consumer/survivors in the mental health system. The goals are to reduce isolation and stigma; to increase accessibility, support reintegration and promote participation. The basic work of MHRC is peer support. Trained staff and volunteers offer one on-one support on a call-in or walk-in basis. Advocacy is also a primary role and is done on an individual basis and systemically. Those who attend are invited to become members of MHRC at no cost and to take advantage of the many services available: a resource library, computer, internet, telephone and support for the use of these. A newsletter, "The Rights Stuff", is mailed to members every three months. It includes stories of interest to mental health consumers and a calendar of events for the upcoming months. There is usually one planned activity each day (games, crafts, karaoke, walks). The newsletter is also available at www.mentalhealthrights.ca and e-mailed to subscribers who are mental health, addiction and community partners as well as any other interested parties who request it. Peer Support Training is offered to persons with lived experience who would like to help others in a supportive way. Members are welcome to come and stay for as long as they wish during the open hours and are encouraged to engage in conversation and develop friendships with other consumers who attend. Members are required to be respectful to everyone present. MHRC tries to be responsive to the needs of consumers and with help from the city of Hamilton, offer a cool place in the summer and a warm place in the winter. Some warm clothing is also available when quantities allow. All information about members is kept in strict confidence and statistics are never attached to names. No information about diagnosis is required or kept on any written record.

Improve your coping skills and learn MBSC. Mindfulness teaches us to follow our thoughts through to their roots. It helps us to rid our body of internal noise and destructive automatic thinking. Mindfulness can set the mind at greater ease and allow us to regain insight and control of our life. An end goal is that participants learn to recognize their own troubled thinking patterns. When we are aware of our self-destructive thinking and the distractions that take us off task, impairing our concentration and sabotaging our relationships, we can then face them, challenge them and change them. The primary benefit is that participants will improve their focus and concentration and begin to heighten their self-awareness. The day consists of individual and group counselling, outdoor activities, healthy active living, life skill building, community resources, guest speakers and academics. Upon entry assessments are completed to determine the amount of substances that the adolescents are using. With this information it provides a starting point for the counsellor to work with the individual to start setting goals around making changes in substance use. If the adolescent successfully completes the 16 weeks of program a Healthy Active Living credit will be granted by the teacher on staff.

Day treatment program is available to clients with eating disorders who need more intensive treatment. Clients participate in a half day of schooling in the morning (our teacher is provided by the York Region Board of Education) and a half day of therapy in the afternoon, which may include group therapy, individual or family therapy. Families are required to actively participate in weekly family meetings, as well as weekly family education nights. Typical goals for our day treatment clients may include weight restoration, symptom interruption, normalized eating and interruption of negative thinking patterns that support the problem. Model of service is interprofessional.

All outpatient programs have moved to virtual means of servicing their patients wherever possible.

Only urgent cases, or patients requiring urgent medical components ie. Injections, etc., are being seen face to face at this time.

The program is sponsored by CMHA, and offers on-site, community-based services in Waterloo Region and Wellington County. The 1st Step Program is based on a recovery model, and as a result, the determination of scope and intensity of services is based on the needs of the individual and his/her family. The District of Nipissing Early Intervention in Psychosis program at the Mental Health Clinic serves individuals age 16-35 who are experiencing a first episode of psychosis and who have not been treated with antipsychotic medication for more than one year. The program involves families from the very beginning. Clinical services include intake/screening assessment and intensive case management. The following interventions are offered to support recovery: medications, education about medications, education for psychosis, supportive talk therapy, assistance with work/school/social and leisure activities, education and support to family/friends. Research has found that early intervention and treatment leads to less need for hospitalization, more rapid recovery, less family and social disruption, improved maintenance of social skills and supports, reduced risk of relapse and faster recovery.

The National Eating Disorder Information Centre (NEDIC) is a non-profit organization founded in 1985 to provide information and resources on eating disorders and food and weight preoccupation. The Centre's telephone helpline and Instant Chat provides information on treatment or support for eating disorders in Canada.

One of our main goals is to inform the public about eating disorders and related issues. The Mental Health Programs and Services division of the Ontario Ministry of Health has mandated NEDIC to provide information about issues related to eating disorders, including up-to-date information on what treatment is available.

This includes psychogeriatric resources to address the assessment, management and transitional care needs of the client with the goal of reintegration to community or long term care. The TrueNorth opioid support program is dedicated to helping people who are experiencing problems related to opioids.The term "opioid" refers to a group of analgesics (painkillers) that includes medications such as codeine, heroin, morphine, fentanyl, oxycodone, hydromorphone etc. These drugs can be extremely powerful pain medications, but they can also be very dangerous and highly addictive. In Ontario, twice as many are dying from opioid as are dying from HIV.
There are a lot of misconceptions out there in terms of opioids. One of the biggest myths is that only "recreational" and/or "street" users of opioids are at risk of negative consequences from their opioid use. This is not the case at all. In fact, more than half the patients we see were started on opioids for pain and have never bought an opioid on the street. The treatment of opioid-related problems is often very complex and challenging. For this reason, our team uses a multidisciplinary team environment that includes counselors, psychiatrist and addiction medicine specialists. We also take pride in using state-of-the-art technology and incorporating the latest advances in health care and opioid research.As with all of our TrueNorth programs - Our goal is to provide all of our clients with first-rate care in an environment that is safe, respectful and confidential. Halton & Mississauga RAAM (Rapid Access Addiction Medicine) Clinic is an outpatient addiction medicine clinic that provides expert assessment, brief counseling, and medication-assisted treatment options for stimulant use disorder, cannabis use disorder, and other substance use disorders including opioids and alcohol.

Stimulants include substances such as cocaine, methamphetamine (“crystal meth”), and amphetamine (“speed”). Stimulants are highly addictive, and their use can become compulsive and out of control. This can lead to any number of problems affecting family, finances, relationships, or functioning at work or school. Stimulants can also contain contaminants, including the highly-potent opioids fentanyl and carfentanil which can cause unintentional overdose and death.

We also see patients with cannabis use disorder. Contrary to popular belief, cannabis can be an addictive substance. Approximately 1 in 11 (9%) of adults who use cannabis will become addicted to it, and this number rises to about 1 in 6 (17%) for those who started using cannabis as a teenager. Frequent cannabis use can have negative long-term consequences, which is particularly risky in youth. Some of these possible long-term consequences include psychosis, impaired attention and memory, decrease in IQ, and altered brain development.

Although addiction counseling and mutual support are currently the mainstays of treatment for stimulant and cannabis use disorders, there are several emerging evidence-based treatment options which can be considered in combination. Halton & Mississauga RAAM Clinic offers expert medical assessments and brief counseling to patients with these conditions, and a comprehensive care plan will be developed, which includes a discussion about potential evidence-based medication treatment and counselling options. Our services are 100% covered by OHIP.

The Rapid Access Addiction Medicine clinic (RAAM), funded by the NELHIN, provides an opportunity for individuals with substance use issues to access safe, non-judgmental specialized addiction medicine in a timely manner. While appointments are preferred, this low barrier model allows for drop-ins and self-referral. The navigator is available 5 days a week.

This multidisciplinary team combines addiction medicine (physicians who specialize in substance use disorder) with the psycho-social supports that many individuals struggling with substances require to improve their quality of life. In addition to medical intervention, assessment, treatment referral, and case management (housing supports, financial support, etc.) are provided.

Once patients are stabilized, they are referred back to primary care for long-term follow-up; the clinical team offers ongoing support to family physicians through re-assessments and addiction specific consultations. In this model, addiction specialists, hospital staff, family doctors, and community service agents work with the individual to provide seamless and accessible care.

Eligibility is assessed at contact with emphasis on opiates, alcohol and sometimes other substances.

This outreach program is centered in Georgina, Ontario. Services are now offered in Newmarket as well. It is offered 24 hours a day, seven days a week and supported by an experienced team of health care professionals consisting of nursing, occupational therapy, social work, a peer support worker and a psychiatrist. The objective of the ACTT is to provide intensive services for people with a severe mental illness who reside in Georgina. The primary goals of ACTT are: lessen or eliminate the debilitating symptoms of mental illness each client experiences and to minimize or prevent recurrent acute episodes of the illness; meet the client's basic needs and enhance their quality of life; improve functioning in adult social activities and employment roles; lessen the family's burden of providing care; decrease the number of hospital admissions, re-admissions and bed days, emergency visits and crisis service contacts. The ACTT delivers treatment, rehabilitation and support services in a client's choice of environment. Model of service is psychosocial. Average length of stay is seven years.

ACTT and PACTT continue services, telephone visits and face to face visits for medication drops, injections and assessment of high risk clients.
All outpatient services have to some degree been modified and where possible are providing virtual support, exceptions to this are identified above. Virtual includes phone, video conferencing, OTN etc.

New referrals are being triaged and some are being seen via virtual means.

Assertive Community Treatment (ACT) is client-centered, recovery-oriented mental health service delivery model that facilitates community living, psychosocial rehabilitation, and recovery for persons who have the most serious mental illnesses, have severe symptoms and impairments, and have not benefited from traditional outpatient programs. ACT Timiskaming serves clients throughout the district of Timiskaming. Important characteristics of the program are: ACT serves clients with serious mental illness that are complex and have devastating effects on functioning, and who, because of the limitations of traditional mental health services, may have gone without appropriate services. Consequently, the client group has often been unfairly thought to resist or avoid involvement in treatment. ACT services are delivered by a group of multidisciplinary mental health staff who work as a team to provide the majority of the treatment, rehabilitation, and support services clients need to achieve their goals. The team is directed by a team leader and a psychiatrist and works in shifts to cover 24 hours per day, seven days a week to provide intensive services (multiple contacts may be as frequent as two to three times per day, and are based on client need and a mutually agreed upon plan between the client and ACT. The services are individually tailored with each client and address the preferences and identified goals of each client. The approach with each client emphasizes relationship building and active involvement in assisting individuals with serious mental illness to make improvements in functioning, to better manage symptoms, to achieve individual goals, and to maintain optimism. The ACT team delivers services in community locations to enable each client to find and live in their own residence and find and maintain work in community jobs rather than expecting the client to come to the program. Seventy-five percent or more of the services are provided outside of the program offices in locations that are comfortable and convenient for clients. ACT services are delivered in an ongoing rather than a time-limited framework to aid the process of recovery and ensure continuity of a caregiver. The model of service is assertive community treatment.

Community Outreach and Support Team (COAST) is a multidisciplinary team with social workers, nurses, psychiatrists, community support and peer support workers. The program offers Assertive Community Treatment and was established to provide services to adults experiencing serious and persistent mental illness. The function of COAST is to assist people in moving towards recovery and to facilitate a higher level of independence in the community. The involvement of family, friends and other supports is encouraged and welcomed. COAST is a mobile service that provides comprehensive and individualized treatment, housing, medication, and rehabilitation (e.g. employment, finances, cooking etc.). The intensity and duration of service is based on each person's needs and wishes. COAST provides services throughout the entire Parry Sound and Muskoka regions with teams in Parry Sound, Sundridge, and Bracebridge. Model of service is assertive community treatment, psychosocial rehabilitation, and recovery. Average length of stay is 2.6 years.

The Supportive Addiction and Mental Health Housing (SAMH) program in Guelph-Wellington is provided by Stonehenge Therapeutic Community in partnership with the Canadian Mental Health Association Waterloo Wellington Branch. The program is designed to support persons with problematic substance use or concurrent mental health and substance use issues who are homeless, at risk of becoming homeless, or are considered inadequately housed.

Applicants must also demonstrate financial need. Participants are accepted into the Transitional Program for a period of 6 months, and can submit a written request to extend their time in the program for up to a total of 364 days. Extensions will be granted based on the individual’s identified recovery goals and their demonstrated active participation in the program. Services are delivered from an abstinence-based perspective. Transitional housing is available to those who meet the above-noted criteria, who have recently participated in residential or day addictions treatment and are seeking to stabilize an abstinent recovery in the community.

Our units target women with problematic substance use, who are homeless or at risk of being homeless with the aim of obtaining permanent housing. Support is offered by way of counselling, case management, matching individuals to appropriate housing environments including land lord tenant intervention such as conflict resolution where required.

The Addiction Supportive Housing Program provides independent apartment living for those who struggle with substance use and homeless/inadequate housing. The program offers counselling around substance use issues as well as assistance with life skills. Referrals to other programs within the organization or other community resources are provided as necessary. Support is provided using a harm reduction framework that can include abstinence.

Offices at ASYR are CLOSED to the public. Staff will be providing support through telephone and other electronic means.

A CTO is a treatment and service plan which is issued by a physician in consultation and coordination with the consumer and service providers. We provide intensive Case Management services to people with severe mental illness aged 16 or older living in St. Thomas or Elgin County who need help working on skills for living independently. This service is provided through our main office in St. Thomas and through satellite offices positioned in the east and west of the County. Case Managers provide advocacy and one-to-one help and support to people learning how to live with and manage their mental health. Case managers can assist with improving relationships, budgeting, vocational goals, finding housing, and accessing addictions or other support services. Services are client-directed and are based on individual need. Services are provided using a psychosocial rehabilitation model focused on recovery.

Case Coordination provides adult psychiatric treatment and rehabilitation supports. Service plans are individually developed based on client need and comprehensive assessments. Clients are assigned a primary worker. An intensive case management model with goal-specific access to an interdisciplinary team is utilized for individuals who have complex and intensive crisis, treatment and support needs. Crisis supports are collaborated with the addiction, mental health and crisis services - Here 24/7. The service has extensive partnerships with community service providers. Case Coordination provides long-term, individual intensive mental health support for the seriously mentally ill population with complex and concurrent needs. Case Coordination provides service to Guelph and North Wellington. Model of service is recovery focused with psychosocial rehabilitation interventions. Clients have access to an interdisciplinary team based on individual assessment and need. Average length of stay is until disengagement and is based on a long-term need and client preferences.

The multidisciplinary team of psychiatrists, OT, RT, RNs, RPNs, SW, an addiction therapist, a peer support worker, a personal support worker, and case workers provide individualized care plans and group programming to serve the diverse treatment, social, and recreational needs of our clients. The Community Support Program (CSP) offers long term case management to youth aged 16-24 with a serious mental illness. Service includes individualized, flexible and comprehensive planning. Community Support Workers offer individual and group counselling, support, crisis intervention and linkage to other services and assist in providing a seamless transition to adult mental health services. Model of service is recovery.

We are continuing to provide service to our clients remotely via phone check in and brief sessions with potentially some access through video technology. Workers are not providing face to face contacts.

A periodic response may be needed to help individuals maintain their independence. Model of service is psychosocial/recovery. Specialty case management services are available. Average length of stay is zero to 60 days for short term and over 60 days for long term. Up to 6 months and then the need is re-assessed.

Housing First Intensive Case Management (HFICM) provides support to individuals in Toronto who are experiencing homelessness, and are living with mental health issues. It is our belief that all people deserve housing, and that adequate housing is a precondition for recovery. We collaborate with clients in locating housing, and provide them with the support they need to maintain their housing and achieve their goals. Through regular meetings with a case manager (in our offices or in the community). HFICM provides: help to obtain adequate housing; support with making social and community connections that will assist in recovery; supportive counseling, physical assessment, and medication management as needed.

The COPE Mental Health Program of Community Care Durham is a unique community-based program that supports adults with mental health concerns to live in their community. The goal is to assist adults age 16 and up with emotional or mental health concerns to remain in the community and function as effectively as possible while maximizing their quality of life.

Services include support and coping skills groups co-facilitated by trained volunteers and COPE Staff. When needed, the COPE staff will provide one-to-one support on a short-term, focused basis with a goal of moving the client eventually into the volunteer-based services.

An action plan is developed with the client to establish goals and a framework for the supportive relationship and the action plan is reviewed regularly.
COPE offers coping skills and support groups co-facilitated by trained volunteers. The groups include, for example, social-recreation, life skills, family support, depression, and anxiety coping groups. Most groups meet weekly, are open and continuous. A list of Region wide groups is available on the website. Recruitment of volunteers, their training, monitoring and supervision is ongoing. Volunteers receive comprehensive training on the fundamentals of mental health and advanced training on group facilitation.

There is no fee for COPE services.

Our Clubhouse program has turned into a virtual program versus an in person attendance program. All employment services are on hold.

We are offering a daily meal for takeout from 576 Church St. along with delivery for those most vulnerable with complex health issues for Progress Place members and are currently exploring if we have capacity and resources to scale up our meal delivery program.

The Progress Place Clubhouse is a recovery centre for people living with serious mental illness. The clubhouse is a holistic community mental health program that provides a full range of social, vocational, housing and recreational activities. Progress Place provides opportunities to gain confidence through a day program with meaningful activities and opportunities, a range of employment opportunities, supported education, access to safe and affordable housing, and social/recreational programs in the evenings, on weekends and on holidays, health and wellness program, and a peer support telephone line. Model of service is a recovery model based on psychosocial rehabilitation principles. Average length of stay is not limited.

The virtual Walk-In Counselling Service is a brief counselling service that provides immediate access for children, youth, adults, couples and families. Each session can last an hour to an hour and a half. On Monday, individuals complete the intake with our Access workers, have their appointment on Tuesday with counsellors from Algoma Public Health, Sault Area Hospital, John Howard Society, Algoma Family Services, and then they are contacted for feedback/evaluation by our Access workers on Wednesday. This is a partnership between Canadian Mental Health Association, Algoma Public Health, Algoma Family Services, John Howard Society, and Sault Area Hospital.

Wounded Warriors Canada and Ontario Shores Centre for Mental Health Sciences (Ontario Shores) recognize the need to provide a range of specialized mental health services, including prevention, crisis supports and treatment to First Responders. First Responder Assist builds on the clinical expertise of both organizations and will enhance the culturally specific trauma programs provided by Wounded Warriors Canada.

First Responder Assist provides an inter-professional team dedicated to serving the psychological needs of all Public Safety Personnel/First Responders in the province and offers prevention services, crisis support, and treatment in the form of individual and group therapy.
