The Early Psychosis Intervention program is a partnership between Brant Community Healthcare System, Community Addictions, Mental Health Services of Haldimand-Norfolk, and St. Leonard's Community Services to provide a regional intensive mental health service that involves assessment, consultation, education, and treatment for people who are experiencing symptoms of early or first break psychosis and their families. Individuals are eligible to receive service for up to three years. The interdisciplinary team helps individuals and their families identify concerns, goals, and develop plans to support wellness and recovery. To provide service as close to home as possible, the team has multiple sites throughout Brant-Haldimand-Norfolk Counties through the generosity of community partners (New Credit, Townsend, Simcoe, Dunnville, and Brantford). The team includes: Psychiatrists, Registered Nurses, Registered Practical Nurses, Occupational Therapists, Masters in Social Work, who also have access to Neuropsychologists and Recreational Therapists. This program also provides Family Education/Support for those individuals or family members of individuals who are experiencing Early Psychosis but are unwilling to enter the program.

The treatment philosophy will employ both harm reduction and abstinence strategies, determined by client need. The treatment modality will primarily be cognitive-behavioural. Participation in community self-help options will be encouraged but not mandatory. Case management involves coordinating all services received by clients, reviewing and revising treatment plans at regular intervals, aftercare and follow-up. Staff has also been certified in the Level 3 - Triple P program (Positive Parenting Program) as well as the Stepping Stone program for families with a child who has a disability. A co-dependency educational group "Disease to Please" is also available. Naloxone kits and training may be available for current clients.

SHIP's support within housing services assist clients with mental health and addictions in a variety of settings to work towards recovery goals, and living independently in the community. Having accepted a housing unit and upon signing the tenancy agreement with SHIP housing, clients are then assigned to services that matches their identified needs. Counsellors in Dual Diagnosis work with individuals who have a developmental delay and a serious mental illness. Support to those in the Dual Diagnosis stream includes support, education and life-skills training. This supportive assistance includes the development of customized learning tools to meet the needs of individual learning styles. Recovery Residences are home to SHIP clients who require a more supportive living environment and additional support in order to work toward their goals of independent living. The Child & Youth Mental Health Eating Disorder (CYMHEDO) Clinic is a comprehensive, interdisciplinary and specialized program that provides assessment and treatment for children and youth with eating disorders. The team consists of a Child Psychiatrist, a Pediatrician, a Behavioural Therapist, a Registered Dietitian, a Social Worker (MSW), and a Nurse Practitioner. The CYMHEDO team works with the child/youth and their family to create specific treatment goals to correct medical complications, restore body weight and promote normal growth and development, regulate eating patterns, and treat underlying mental health issues. Treatment is family based and includes Family Therapy, Family Based Dialectical Behavioural Therapy (when appropriate), Individual Therapy, nutritional education and support, health monitoring, and medication monitoring. The Eating Disorders Unit is a 12 bed unit for children and adolescents struggling with an eating disorder. It provides an interprofessional model of care with a focus on recovery and rehabilitation. There is a pre-admission process, post-discharge support and planning for transition home. The unit includes individual patient bedrooms, kitchen and dining room and lounge area. There are also group rooms and spaces for patient, family and group therapy. Parent and family involvement is a key component of the clinical programming.

During their time at Ontario Shores, patients will also have access to teachers who can assist with their education needs.
Programming includes: Intensive individual therapy; group and family therapy; Psychoeducation; Meal support, nutrition and eating rehabilitation; Psychopharmacologic treatment; Schooling; Treatment for co-morbid disorders. Modalities used include; CBT, DBT, EFT and FBT. Inclusion criteria:

The client must be between 12 years of age and 17 years of age and the referral must be submitted at least three months prior to their 18th birthday; All treatment options for eating disorders available in the client's community have been unable to assist the patient reach recovery; The client is unlikely to benefit to a less intensive treatment other than residential care; Serious eating disorder symptoms continue after less intensive treatment in the community have been undertaken. i.e. day treatment and outpatients if available in community; Referral source is able to commit to recommended follow-up after the teen has completed the Ontario Shores Eating Disorder program; Referral source is able to accept or arrange medical transfer should the patient become medically unstable; Patient be medically stable, orthostatic vital signs, blood work, and EKG must be stable.

Residential treatment is provided through adherence to an evidenced based clinical care pathway that equates to a 105 days length of stay.

Our clients have a Global IQ of 70 or less (i.e., below 3rd percentile) and/or exceptionally low range of adaptive ability. We strive to not duplicate services that are currently providing psychiatric care for the individual or available in the client's home area. The Developmental Dual Diagnosis Outpatient Program is a community-based mental health service. The program provides assessment, treatment planning, and education to clients, families and other support services using a team-based approach. Our team members include psychiatrists, nursing, and occupational therapy. Our staff is able to provide assessment in the client's home setting if needed (within the HNHB LHIN). We strive to collaborate with other service providers connected to our clients and family. We have the ability to provide translation services when needed.

An outpatient, community-based program for people experiencing early stages of psychosis who reside in the Halton Region. The Phoenix Program is a clinical program affiliated with Halton Healthcare Services, Joseph Brant Memorial Hospital, North Halton Mental Health Clinic, ADAPT and the Schizophrenia Society of Ontario. Our services include: rapid psychiatric assessment, treatment based on individual concerns/goals including education about psychosis, assistance with work, school, social and leisure activities, specialized support to family members and friends, addictions counselling, and introduction to other community programs and planning for management of illness after discharge from the program. The staff available to support clients include: family educators, nurses, occupational therapists, psychiatrists, social workers, addiction clinicians and peer support/recovery mentors. Model of service is recovery-oriented, emphasizing: client choice, flexibility in services, individual supports, and the importance of families, significant others and communities. This approach also considers the impact of factors such as poverty, poor housing, unemployment and stigma on people with mental illnesses. The early intervention program is consistent with this approach by maintaining a culture of hope and excitement about the possibilities for the future. Average length of stay is three years. Assessment involves a biopsychosocial interview and the completion of assessment tools. Assessment and feedback is generally completed in three sessions, and areas of exploration include gambling, substance use and mental health. Assessment tools commonly administered include the South Oaks Gambling Screen, the Ontario Standardized Assessment Tools, the Substance Abuse Subtle Screening Inventory (Adult and Adolescent), the Beck Depression Inventory, the Folstein Mini Mental State Examination, and others as indicated. The client is at the centre of the entire process. When providing community based treatment, commonly utilized treatment models include motivational interviewing, Cognitive-Behavioural Therapy, Solution-Focused Therapy, and Narrative Therapy. All clinical staff members are cross-trained in addictions and mental health, so clientele with multiple issues receive continuous service with one counsellor. Treatment services are generally provided once per week for one hour. Sessions are typically conducted at the Red Lake or Ear Falls offices with accommodation for special situations (e.g., mobility problems, etc.).

We offer a variety of shorter term and longer term housing options to support people living with and recovering from mental illness. Length of stay is often determined by your personal needs. We’re here when you need us the most.

If you are living with mental health concerns you may face barriers to finding and keeping safe, affordable housing. We hope to reduce those barriers so that your housing doesn’t get in the way of getting and staying well.

We offer support to secure housing throughout Strathroy and Middlesex County for individuals who have a history of homelessness, and do not receive any other formal community supports. Support is self-determined and offered 1:1 in your apartment, out in the community and through a variety of groups. Support is provided intensively for the first 12 months, then as needed up to 2 years, and permanently for those who require long term support. Individuals are responsible for paying their own rent using shelter allotment from ODSP, OW, or another type of income. Staff are committed to helping individuals live independently, improve their quality of life and achieve a sense of community belonging.

The treatment models used in recovery support are cognitive-behavioural, reality, rational emotive therapy, and we teach the 12 Steps of Al-Anon. The program provides a 26-week cycle of sessions that includes a review of components in the seven-day program. Length of stay may be longer than six months if desired by client.

The Rapid Access Addiction Medicine clinic (RAAM), funded by the NELHIN, provides an opportunity for individuals with substance use issues to access safe, non-judgmental specialized addiction medicine in a timely manner. While appointments are preferred, this low barrier model allows for drop-ins and self-referral. The navigator is available 5 days a week.

This multidisciplinary team combines addiction medicine (physicians who specialize in substance use disorder) with the psycho-social supports that many individuals struggling with substances require to improve their quality of life. In addition to medical intervention, assessment, treatment referral, and case management (housing supports, financial support, etc.) are provided.

Once patients are stabilized, they are referred back to primary care for long-term follow-up; the clinical team offers ongoing support to family physicians through re-assessments and addiction specific consultations. In this model, addiction specialists, hospital staff, family doctors, and community service agents work with the individual to provide seamless and accessible care.

Eligibility is assessed at contact with emphasis on opiates, alcohol and sometimes other substances.

Transitional Housing Serenity House operates 24 beds in 3 separate transitional homes located in Ottawa, to support those men who have successfully completed treatment in a residential facility. A prospective applicant must move directly from treatment into Transitional Housing. There will be a screening assessment; if the participant fits all of the criteria and is interested, there will be an interview. The homes house 8 men each and provide a safe environment free from alcohol and drugs where people can start to put into practice skills learned in treatment. Best practices suggests at least one year in a drug-free environment for better long-term results; men are encouraged to set short and long-term goals and to develop skills needed to become self-sufficient and return to the work force with a strong commitment to on-going abstinence from addictive substances. This service fills a much needed gap in the community as it provides temporary housing and serves to transition the men from treatment to eventual permanent and affordable housing. Each person has his own, fully furnished room while sharing common areas with 7 other men. Each is responsible for buying and preparing his own food in an environment where staff offer support services as needed. Men learn to overcome those challenges that have prevented them from enjoying a full, healthy and happy life. Many form friendships that will last a lifetime and that offer a great sense of belonging in community. Help is available to ensure each person has affordable housing prior to leaving. This service helps many individuals who are at risk of homelessness in becoming stably housed.

This is an intensive, home-based program that supports parents as they learn to use positive parenting strategies that promote healthy family relationships. Parents work together with a child and family therapist and child and youth counsellor to identify therapeutic goals that best meet their family’s needs. Ka-Na-Chi-Hih is a 12-bed long-term treatment program for First Nations males between 18 and 30 years old, throughout Canada, whose lives are affected by solvent abuse. The holistic program uses traditional and contemporary models of treatment that embraces the emotional, mental, physical and spiritual components that make one whole. Educational program is offered in both phases with a special education teacher on-site. The program currently consists of two treatment phases. Phase I: The "Core Group Program" is designed to educate the clients in addition to one on one counselling sessions. Phase II: The "Individualized Treatment Plan" is geared towards meeting the client's specific needs. The initial assessment is completed by the referring agent, the information followed by pre-treatment interview(s) will determine if the participant is suited for the treatment program. A voluntary commitment and a willingness to participate in the healing process are required. This program is for a diverse population of individuals, carrying a wide set of diagnoses. Many have developed unhealthy coping patterns, are frequently in crisis, and often struggle to achieve their goals due to a lack of structure and purpose in their daily lives. Many have been diagnosed with multiple psychiatric illnesses, have had frequent hospitalizations, or have found that treatments in conventional settings were not enough to keep them well. Following a comprehensive assessment, a care plan is customized according to the individual's treatment goals and tolerance level. The care plan emphasizes a combination of group psychotherapy, education and skill-building sessions, recreational activities, along with occasional one-to-one sessions with interdisciplinary team members. Medical treatments, including medications and electroconvulsive therapy, are used as deemed appropriate. Treatment is conducted with an interdisciplinary team consisting of physicians, nurses, social workers, chaplain, occupational therapist, recreation therapist, horticultural therapist, pharmacist, addiction counsellor, neuropsychologist, and dietician.

STC offers a five-to-six month residential treatment program for chronic and acute substance abuse issues that is based on a progressive Therapeutic Community model of treatment. The program is holistic with a multi-disciplinary approach (biopsychosocial, cognitive behavioural, stages of change, psychodrama, experiential therapy, and psycho-educational). It is a highly structured program that is group-based with self-help and mutual-support components. Beds are continuous intake.

There is 24-hour support staff. Residents must participate and abide by community guidelines. During the first week of Orientation there is no off-site communication permitted; these privileges increase as residents progress through the program. Support coordination is provided for residents on Opioid Substitution Therapy (OST). Residents participate in a weekly OST clinic with a physician via Ontario Telemedicine Network (OTN) and submit urine screening, and bi-weekly OST psycho-educational group.

Bi-weekly support groups and individual counselling sessions are offered. The Residential Support Counsellor works with the Admissions Coordinator to ensure a smooth transition of residents' Opioid Substitution Therapy services (physician, urine screening, pharmacy etc.) from the community into the residential program. Discharge planning is supported for a smooth transition of services from treatment back into the community through referral and support. Smoking cessation is available through collaboration with the Guelph Community Health Centre and the Smoking Treatment for Ontario Patients (STOP) program. Friends and family are invited to attend the Friends and Family Support Group, held on the third Saturday of every month. Friends and family can attend the support group while their loved one is in treatment and for two additional years provided that their loved one completes a minimum of three months of the residential program. The Peer-Led Overdose Response program was inspired by the award-winning AnchorED Program in Rhode Island. This program matches Peer Recovery Coaches with those who have overdosed by meeting them at the hospital Emergency Department to offer harm reduction and recovery support to them and their family and friends.

Recovery Coaches will provide information about what will happen at the hospital, as well as education related to substance use, overdose, harm reduction, and resources that are available within the community.

Peer Recovery Coaches inspire hope through sharing of their own lived experience of addiction, overdose and recovery and provide support for the first 10 days of transition back into the community. The goal of this program is to reduce repeat overdoses, reduce harm from substance use and educate and support family and friends.

The TrueNorth opioid support program is dedicated to helping people who are experiencing problems related to opioids.The term "opioid" refers to a group of analgesics (painkillers) that includes medications such as codeine, heroin, morphine, fentanyl, oxycodone, hydromorphone etc. These drugs can be extremely powerful pain medications, but they can also be very dangerous and highly addictive. In Ontario, twice as many are dying from opioid as are dying from HIV.
There are a lot of misconceptions out there in terms of opioids. One of the biggest myths is that only "recreational" and/or "street" users of opioids are at risk of negative consequences from their opioid use. This is not the case at all. In fact, more than half the patients we see were started on opioids for pain and have never bought an opioid on the street. The treatment of opioid-related problems is often very complex and challenging. For this reason, our team uses a multidisciplinary team environment that includes counselors, psychiatrist and addiction medicine specialists. We also take pride in using state-of-the-art technology and incorporating the latest advances in health care and opioid research.As with all of our TrueNorth programs - Our goal is to provide all of our clients with first-rate care in an environment that is safe, respectful and confidential. Outpatient assessment treatment and management of youth up to 18, who are either using or at risk for misusing alcohol or other drugs. We also see parents and siblings if indicated. Outreach, teaching and research components, with harm-reduction, open-ended format complimenting treatment. Will support youth with process addictions. Average program length varies depending on the client.

This program is an academic program that gives youth the opportunity to work towards secondary school credits with a TDSB teacher following Ministry of Education curriculum. The program includes daily treatment groups, life skills instructions, ongoing access to healthcare, individual counselling and support.

Mainstay's rent supplement program is geared towards individuals who are living with problematic substance use issues and who are high users of emergency services. Through the rent supplement program, singles and women-led households can access 109 units and follow-up support. Units are available in dedicated or mixed-income buildings.

Mainstay's Supportive Housing Workers provide site support and community development within these buildings. All tenants have the opportunity to be part of Mainstay's ongoing community development activities, and if a situation arises where it appears that the tenant's substance use issues are impacting their ability to maintain housing, site support would be stepped up to ensure the tenant was linked to intensive supports. This would include Mainstay's Supportive Housing Worker providing individual support until the tenant was connected to more intensive external support.

The TrueNorth opioid support program is dedicated to helping people who are experiencing problems related to opioids.The term "opioid" refers to a group of analgesics (painkillers) that includes medications such as codeine, heroin, morphine, fentanyl, oxycodone, hydromorphone etc. These drugs can be extremely powerful pain medications, but they can also be very dangerous and highly addictive. In Ontario, twice as many are dying from opioid as are dying from HIV.
There are a lot of misconceptions out there in terms of opioids. One of the biggest myths is that only "recreational" and/or "street" users of opioids are at risk of negative consequences from their opioid use. This is not the case at all. In fact, more than half the patients we see were started on opioids for pain and have never bought an opioid on the street. The treatment of opioid-related problems is often very complex and challenging. For this reason, our team uses a multidisciplinary team environment that includes counselors, psychiatrist and addiction medicine specialists. We also take pride in using state-of-the-art technology and incorporating the latest advances in health care and opioid research.As with all of our TrueNorth programs - Our goal is to provide all of our clients with first-rate care in an environment that is safe, respectful and confidential. All of our services are covered by OHIP.

Mom and Kids Too is a seven week substance use program for pregnant and parenting women, and their children aged zero to six. The goal is to increase attachment between mother and child and to improve parenting. This seven week program is offered three days per week to support women's needs as mothers in addition to their substance use concerns. The flexible schedule allows mothers to attend to other obligations such as doctor appointments, employment programs, access visits, etc.

Cota Health's Hostel Outreach Program (HOP) provides individualized support to men who are homeless or under-housed and are living with a serious mental illness. HOP Case Managers work collaboratively with their clients to provide client centred support aimed at assisting them to access the basic necessities of life and move toward living active and fulfilling lives within the community. The HOP services are committed to being flexible and responsive to the unique needs and desires of each client.

The supports offered via this program typically include: outreach supports and the development of a trusting relationship between the client and the service provider; comprehensive assessment; support in accessing basic necessities such as personal identification, food, clothing, shelter and income support; collaborative development of a personalized pre-determined safety plan; collaborative development and implementation of an individualized recovery plan and assistance to achieve/maintain health and wellness.

Also we provide direct support in the areas of: education about the nature of mental illness; support and education to the care givers and/or person's support network, where appropriate; development of self-management strategies; encouragement, emotional support and motivation; development of personal support networks; development/maintenance of abilities in the areas of general life skills; referring to and accessing other desired/needed community and primary health care services.

The Outpatient Adult Day Treatment and Rehabilitation Program is for individuals who have experienced serious and persistent mental illness. Consideration is given to individuals with a mood or anxiety disorder who have significant functional disabilities. Once a referral has been made, an initial assessment will be set up to determine eligibility and discuss treatment goals. Model of care is a biopsychosocial framework and recovery model with therapy being offered in a variety of individual and group processes with psychoeducational topics such as: You and your recovery, Skills for Everyday Living, Medication, Addictions Awareness, Goal Setting, Stress Management, Healthy Lifestyles, Social Skills and other health matters. The team is interdisciplinary and consists of the following disciplines: psychiatry, nursing, occupational therapy, social work, dietary, and pharmacy. The focus of the program is to help individuals decide how best improve their everyday life in the community and to assist with linkages to appropriate community resources.

We recognize the interdependence of spiritual, emotional, mental, physical, social, economic, cultural, linguistic and broader environmental aspects of life in working with individuals with severe mental health problems/illness. The Canadian Mental Health Association (CMHA) Toronto, Mental Health and Justice Initiative provides supportive housing and intensive case management services for individuals with serious mental illness and recent or current involvement with the criminal justice system. CMHA Toronto is one of four lead organizations with service and supports accessed through the Unit Registry housed by LOFT. The Unit Registry also provides information on eligibility criteria, program descriptions, unit locations and descriptions and contact numbers. Model of service is intensive case management. Average length of stay is indefinite.
