Crisis Intervention

Program Description

The Four County Crisis Team provides professional crisis intervention support directly to community members within the four counties of Haliburton, Northumberland, Peterborough and the city of Kawartha Lakes through a local toll-free phone line 24 hours a day. Mobile Crisis Intervention Workers provide assessment, supportive counselling, intervention and referral to community resources as appropriate. The team provides support both through telephone intervention and outreach. Those with longer-term needs may be assessed and referred to one of four short-term case managers of Four County Crisis to receive up to 12 weeks of intervention and support in their community. Model of service is mobile and telephone support, based on a psychosocial model. Average length of stay is not limited for telephone support and up to 12 weeks of case management for those who qualify.

The Crisis program also provides the services of case managers who specialize in supporting individuals living with Intellectual Disabilities, Behavioural issues and /or Concurrent Disorders.

An Integrated Outreach Program with Police and the local Peterborough Hospital provides services of a mental health worker who can provide a mental health assessment on site with police vs. an emergency room visit. Supports and referrals are made to services to assist individuals to remain in the community. This Outreach Program Operates between the hours of 11am and 7pm, 5 days per week.

Hospital 2 Home case managers( 2 ) are in both the Peterborough and City of Kawartha Lakes
Hospital(s) and communities and provide transitional support ( up to 4 weeks) between hospital visit ( in patient or emergency room ) and home, facilitating a more seamless transition and preventing need for frequent emergency room visits. Referrals for these two
programs come through Police or through Hospital.
