Case Management - Mental Health
Case Coordination provides adult psychiatric treatment and rehabilitation supports.
Service plans are individually developed based on client need and comprehensive assessments.
Case Coordination provides long-term, individual intensive mental health support for the seriously mentally ill population with complex and concurrent needs. Clients have access to an interdisciplinary team based on individual assessment and need.
Case Coordination services are accessed through Here 24/7 as well as through Homewood acute psychiatry - regional services.
The criteria for long-term support service includes: assessed fit with priority; indicators for match with long-term support services; residency in Guelph or Wellington county.
Support to people over the age of 65: clients actively in long-term services would not be arbitrarily discharged at age 65 but may benefit from a continuity of support with new linkages related to supports and services for seniors. Clients agree to participate in services, as this program is voluntary.
