Program Description

The Hospital Services Navigator ensures the transition is seamless and the right community, and ensures professional and cultural supports are in place to maximize wellness and health of the individual. Case management related functions are provided including: establishing therapeutic relationships, assessment and referrals, providing cultural supports and crisis intervention, assertive outreach, as well as cultural competency education and advocacy.

This program is designed to help align services for those with multiple hospital visits.
