Program Description

Assertive Community Treatment (ACT) is a community-based model of care for individuals who have severe and persistent mental illness. It reaches out to people assertively where and when they need help directly in their home and in the community. It ensures a response for people who are in crisis, no matter what day of the week. ACT brings a multidisciplinary team of healthcare professionals together to provide highly integrated and individualized services for their clients. ACT gives people a single point of access: one door to comprehensive treatment, rehabilitation and support. Model of service is assertive community treatment.

Access Information

The Sarnia Lambton Mental Health Access Committee. A single point of entry for all community mental health services for individuals 18 to 64 years of age with a serious mental illness comprised of representatives from Canadian Mental Health Association (CMHA) and Bluewater Health Mental Health Program.
