Assertive Community Treatment Teams
Assertive Community Treatment (ACT) is a community-based model of care for individuals who have severe and persistent mental illness. It reaches out to people assertively where and when they need help directly in their home and in the community. It ensures a response for people who are in crisis, no matter what day of the week. ACT brings a multidisciplinary team of healthcare professionals together to provide highly integrated and individualized services for their clients. ACT gives people a single point of access: one door to comprehensive treatment, rehabilitation and support. Model of service is provincial ACT standards.
People interested in Assertive Community Treatment (ACT) are encouraged to discuss it with their current treatment team or physician. Eligibility for receiving ACT services is determined through collaboration with the client, ACT staff, the referral source and other mental health care providers. ACT teams can have waiting lists of varying lengths depending on the availability of ACT services as well as the appropriateness and priority of the referral.
