Program Description

Frontenac Assertive Community Treatment Team (ACTT) is a multidisciplinary team which provides outreach, individualized treatment, ongoing and continuing services, linkages, and includes a monitoring and evaluation component. Assertive community treatment is a client-centred, recovery-oriented mental health delivery model that facilitates community living, psychosocial rehabilitation and recovery for persons who have a serious mental illness, have severe symptoms and impairments, and have not benefited from traditional outpatient programs.

Access Information

Referral forms must be completed and are accepted from all sources, including: family physicians, psychiatrists, family members, social workers or the individual requesting service. All referral forms must be signed by the individual who is requesting service.
