Case Management - Mental Health

Program Description

This person-centered program works with people to determine what is personally important for them to achieve. Team members help individuals to identify meaningful goals and effective ways for each person to reach those goals. Team members work alongside the client, enabling them to exercise their rights and to enjoy a meaningful life in their community while striving to achieve their full potential. The team works to promote health, housing, purpose, and community. The program uses a holistic, strengths-based approach to support individuals towards living a full, independent, and satisfying life.

Access Information

Referrals accepted from all sources. Referrals can be made by calling the Central Intake Team. Individuals must be engaged with their worker towards identifying and achieving goals aimed at promoting recovery.
