Program Description

This program helps inpatients transition back to home, school and the community.

Program provides: medication assessment, diagnostic clarification, crisis intervention and stabilization of the individual by an interdisciplinary team consisting of a psychiatrist, social worker, nurse, child & youth worker, three teachers, educational assistant and other disciplines as required. Patients attending the program will participate in the classroom to promote academic achievement while receiving therapy.

Access Information

Referrals accepted from the inpatient unit, the outpatient department, the Emergency Department, family physicians, paediatricians, schools and the Children`s Aid Society.
Please call for information. The referral form is available on the website. Referrals can be faxed to intake after which an appointment will be scheduled.

Individuals in this program are required to maintain abstinence from substance use while in the transition program. Medical clearance in the form of a doctor`s note is also required in order to be admitted to the program.
