Program Description

The program provides outreach, referral assistance and case management services to adults with mental illness in the Low German-speaking Mennonite community. Support is available in Low German and is culturally-specific to this community. Promotes independence and improves quality of life for persons in this diverse population. Services include: assertive outreach; family support and education; information and education; supports and links to community resources; supportive counselling; person-centered planning; medication support; crisis planning/intervention/prevention; skill development; and advocacy.

Must belong to the Low German Speaking Mennonite community. Referrals come from a variety of sources including doctors, hospitals, family members, other community organizations and self-referrals.
