Program Description

Health Promotion provides health education programs to clients and family members of City Centre Health Care, as well as to people living in the community who may or may not be attached to the agency. Ongoing needs assessments are completed using various strategies including: client and community general surveys, targeted surveys, statistics provided by the local health unit and other community partners. Results highlight areas of concern and gaps in services, assisting agency staff to develop programming that most appropriately meet the needs of the target audience. The determinants of health serve as the foundation for the Health Promotion programs with a consistent focus to foster the best possible health and wellbeing of the population. Program activities and topic areas include education, promotion and prevention topics such as: smoking cessation; health fairs; education; chronic pain & low back pain groups; anxiety group; diabetes wellness; yoga; dance fitness.

Access Information

Individuals without a primary provider (or wish to change providers) and live within the Windsor boundaries of: Crawford Ave, Tecumseh Rd, Central Ave and Riverside Drive; individuals with a moderate to severe mental health illness living in Windsor or Essex County. Attend the agency to complete a new patient registration form and submit to the secretaries at the front desk. Located at 1400 Windsor Avenue, Windsor, Ontario N8X 2L9. Valid health card is preferred
