Program Description

The COPE Mental Health Program of Community Care Durham is a unique community-based program that supports adults with mental health concerns to live in their community. The goal is to assist seriously mentally ill adults to remain in the community, function as effectively as possible and to maximize their quality of life. Services include support and coping skills groups co-facilitated by trained volunteers and program managers. The COPE program manager receives referrals, completes an assessment (OCAN) and initiates the service. When needed, the program manager will provide one-to-one support on a short-term, focused basis with a goal of moving the client eventually into the volunteer-based group services. An action plan is developed with the client to establish goals and a framework for the supportive relationship. The action plan is reviewed regularly. The model of service is primarily based on the recovery model.

Access Information

The COPE program accepts referrals from all sources including self, family, clinicians, etc. Most referrals come from other mental health programs and social agencies. There is no diagnostic criterion other than a mental health problem. Referrals can be made to any of our seven local programs.
