Program Description

City Centre Health Care offers primary care services to meet the physical and mental health needs of individuals and families. The Centre further operates under the collaborative care model where the interdisciplinary team of family doctors, nurse practitioners, and other health care professionals including RPNs, dietitian, therapists, health promoter, physiotherapist, respiratory therapist, and chiropodist take a holistic approach to patient care by addressing both the primary care and the mental health concerns.. The team assesses client need, creates and implements treatment plans that effectively address client concerns. Referrals for on-going primary and mental health support are made according to the treatment needs of the patient. The clinical team offers care using best practice guidelines.

Eligibility criteria: individuals without a primary provider (or wish to change providers)and live within the identified Windsor catchment area of: Crawford Ave, Tecumseh Rd, Central Ave, Riverside Drive; individuals living anywhere in Windsor and Essex County and have a moderate to severe mental health diagnosis. Attend the site to complete a registration form and submit to secretarial staff located at 1400 Windsor Avenue, Windsor, Ontario, N8X3L9. Valid health card is preferred.
