Case Management/Supportive Counselling & Services - Mental Health

Program Description

This service offers individualized, community-based support for people 16 years of age or over suffering from a serious mental illness. Service provided is client directed and aimed at improved quality of life through community integration. Independence is encouraged through supportive decision making. Case managers assist with housing, financial planning, development of a supportive network of family and friends, life skills, involvement in meaningful activities, and linking with other professionals and community organizations. Model of service is recovery and psychosocial rehabilitation.

Access Information

Priority is given to those with a serious mental illness. Brief information is taken by reception and then given to the Intake Worker for initial screening. This is a voluntary service and when agreement for service is ascertained, assignment to a case manager takes place as soon as possible. Support is provided by intake worker until case manager becomes available. If the consumer finds the program is not suitable for their needs, all attempts are made to refer them to other organizations.
