Program Description

This program works with men and women who are homeless and experiencing serious and persistent mental health concerns. The Shared Care clinical outreach service addresses the physical and mental health needs of homeless people using the shelters, hostels and drop-ins in the downtown Toronto area. Team members include: full-time registered nurses, outreach workers, a housing worker, visiting family physicians and visiting psychiatrists as consultants. Shared Care teams work collaboratively with staff from other agencies to deliver respectful and comprehensive care, and facilitate access to resources, enhancing the individual's quality of life. Model of service is recovery/mobile. Average length of stay is 90-180 days.
