Assertive Community Treatment Teams (ACTT)

Program Description

Provide a community based combination of treatment/symptom assessment and support, medication management, intensive case management and individual supportive therapy. The team also provides crisis assessment and intervention, rehabilitation/support, services-vocational, ADL, social, creative outreach and engagement in the therapeutic process by a multidisciplinary mental health team.

Individuals must be diagnosed with a major mental illness and be in need of intensive 3- to 5-day per week, community-based support.

Access Information

Referrals are accepted from a medical doctor, healthcare professional, or by self-referral. All referrals are welcome. However, referrals from outside primary service area are accepted for consultation only.
