Program Description

This program facilitates the development of comprehensive support plans for and with individuals with dual diagnosis with complex needs. In addition, the program aims to create partnerships by bridging relevant systems. The Brokerage project exists and is funded to perform roles and functions that cross sectors including: navigation, increased access, crisis prevention, community development and identify gaps in service for people with dual diagnosis. Model service is brokerage model. Brokers work with individual to develop a support network that meets their individualized support plan. Dual Diagnosis Justice Case Managers also accept referrals for individuals with Dual Diagnosis in conflict with the law. Individuals must be eligible for Developmental Services Ontario (DSO).
