Program Description

The Homes for Special Care (HSC) Program is a long-standing, province-wide, residential program established by the Ministry of Health and Long-Term Care (MOHLTC) for adults with serious mental illness. The local HSC Program has approximately 9 urban and rural Homes across Hamilton and Niagara that offer limited supervision and assistance with activities of daily living. Although HSC Homes are independently owned, the operation and staffing of the homes, as well as compliance with MOHLTC licensing regulations, are all monitored by the program's interdisciplinary clinical staff. The staff ensures that consumer-residents have access to therapeutic & support services, based on the individualized assessments, needs, and preferences of the consumers. The Central South HSC Program also oversees two Skills Centres, one in Hamilton and the other in Vineland. These Centres offer HSC consumer-residents a range of rehabilitation, vocational, recreational, leisure & social opportunities in which to develop or rebuild transferrable skills & meaningful experiences. All services of the HSC Program, whether in-Home, at the Skills Centres or throughout various communities, promote consumer wellness/well-being, enhanced functioning, and engagement within their respective environments. Length of stay is variable and depends on each individual's preferences and needs, with some consumers eventually transitioning to available & suitable supported living.

Access Information

Adults with a psychiatric diagnosis and current or prior psychiatric hospital admission(s), at least eighteen years of age, and voluntarily agreeing to enter Homes for Special Care (HSC) Program. Self-referrals (consumer-applicants) are accepted, as well as referrals from families, mental health and/or health/allied health professionals & service organizations. Applicant must have a psychiatrist and family doctor providing or willing to provide follow up. Compliance with prescribed medication and follow up, and must not be using/abusing alcohol, drugs or other substances. Must not require alternate level of care typically provided by long term care facilities. In order to process a referral for admission to the HSC Program, the following information/tests and completed forms must be provided: authorization for disclosure of personal information (consent); TB and Hep B test results; HSC Program Referral Profile; financial responsibility acknowledgement for tenants of Homes for Special Care; application for reduction in Homes for Special rent and care fees; income verification (notice of assessment from Revenue Canada or copy of cheque stubs from income sources such as ODSP/employment/pension); medication record; a recent psychosocial or other relevant assessment(s); and psychiatric history record.
