Program Description

Case managers provide a moderate level of support to consumers throughout the Niagara region that includes engaging consumers and/or families to provide emotional support and mental health education, daily life counselling, and referral for appropriate clinical treatment, legal advocacy and discharge planning. Model of service is psychosocial rehabilitation.
