Case Management - Mental Health
Community Treatment Order (CTO) program assists individuals by providing treatment, care and supervision in the community instead of a longer stay in hospital or as an alternative to being hospitalized. The CTO plan includes intensive case management supports and identifies what the individual and their supports (e.g. family, friends, and health care professionals) must do to help them stay well and out of hospital.
A psychiatrist can refer by using the Community Treatment Order (CTO) for their patient and involving the coordinator and rights advisor. At this time this is usually the point of entry for the CTOs.
