Program Description

This program assists individuals who are homeless, or at risk of being homeless, to find affordable housing. It also provides case management services (individualized budgeting, money management, shopping, helping to maintain housing, etc.). Support services emphasize client choice, skills development, and community access and consumer rights. The program is for individuals 18 years of age and up who have a chronic mental illness. Model of service is psychosocial rehabilitation, recovery-based.

Access Information

A "Request for Service" form is available from various programs and services in Thunder Bay, including the CMHA. Anyone may refer, including families, clients, psychiatrists, etc. To qualify for the program, the client must agree to be an active participant in the recovery/ rehabilitation process. The client must be homeless or at risk of homelessness.
