Program Description

In an attempt to improve the mental health and independence of seniors, the team provides assessment, consultation, support and counselling to those seniors living in their homes, community residence, or in a long-term care facility. Support and education are also provided to families and staff. We work collaboratively with family physicians, community organizations and health care professionals.

Access Information

Eligibility: must be at least 65 years of age, have a significant mental health/emotional problem and reside in the County of Stormont-Dundas-Glengarry or Akwesasne. Individuals seeking help can present themselves in person to our main office in Cornwall or call. Referrals can also be made by a community organization or service, physician or other health care professional, or a family member.
