Program Description

The Mental Health Youth Patient Navigator is available to provide a blanket of support approach of the best interest to families, the community, and service providers, to ensure that the individualized needs of youth are addressed with suitable supports. This role is crucial in helping all Aboriginal youth reconnect with the community. The intent of this service is to develop and guide youth to engage in a support system similar to a traditional family unit.

Access Information

Self-referrals are preferred, and referrals from organizations, professionals, and family members are also accepted. All calls will be returned within 72 business hours.
