Case Management/Supportive Counselling & Services - Mental Health

Program Description

Mental health community support with emphasis on shorter-term support as well as specialized support for those with justice involvement. Supports are available to individuals who have a diagnosed or diagnosable mental illness. Supports are available to individuals who are experiencing ongoing difficulties. Model of service is psychosocial rehabilitation. Average length of stay is up to a year.

Access Information

The organization accepts self-referrals and professional referrals. Calls may be placed to our central information and referral line to speak with a mental health centre staff person. Inquiries may also be made in person. Intake referral form can be faxed and will be directed appropriately.
