Program Description

St James Town Outreach Program (STOP) works with vulnerable adults age 55+ years with mental health issues or concurrent disorder and no visible community support living in the St. James Town community. Outreach is done to engage isolated seniors in the four Toronto Community Housing Corporation (TCHC) buildings in the community. The presenting issues of the client population include: lack of medical or psychiatric support, unaddressed or complex health issues, social isolation, pending eviction, infestation or extreme clutter in the unit, etc. The service is provided for 9 months and includes linkages to ongoing supports.
