Program Description

Provides support to individuals in Toronto who are experiencing homelessness, and are living with mental health issues
Supports include:
- help to obtain adequate housing
- support with making social and community connections that will assist in recovery
- supportive counseling
- physical assessment
- medication management as needed

Access Information

Referrals are accepted from all sources. Intake is via The Access Point.
