Program Description

Community Outreach and Support Team (COAST) is a multi-disciplinary team with social workers, nurses, psychiatrists, community support and peer support workers. The program provides intensive case management for individuals experiencing serious and persistent mental illness and developmental disabilities. The function of COAST is to assist people in moving towards recovery and to facilitate a higher level of independence in the community. The involvement of family, friends and other supports is encouraged and welcomed. COAST is a mobile service that provides comprehensive and individualized treatment, housing, medication, and rehabilitation (i.e., employment, finances, cooking). The intensity and duration of service is based on each person's needs and wishes. COAST provides services throughout the entire Parry Sound and Muskoka regions with teams in Parry Sound, Sundridge, and Bracebridge. Model of service is psychosocial rehabilitation, and recovery.

Access Information

All Consumers Must Meet the Following Criteria: 1. Have a recognized developmental disability of a mild to moderate severity; diagnosed with or exhibits behaviours that are attributable to a serious, persistent, and complex mental illness with a priority being given towards pre-existing psychotic and bi-polar disorders; Individuals/substitute decisions maker must have provided consent to the referral; have the capacity to reside within a community setting but requires intensive community-based supports to do so. 2. All Consumers Must Experience Complex Deficits in Three or More of the Following Areas: Communication, Self-Care, Self-Direction, Social Skills, Community Use, Home Living Skills, Health and Safety, Work, Leisure, Functional Academics. 3. In Addition, The Consumer Must Meet Two or More of the Following: Is at risk for repeated or prolonged institutionalizations within a health care or correctional facility. Has had 3 or more psychiatric hospitalizations within the last 2 years or has been detained for more than 60 days within a psychiatric facility within the same time period. Is socially or geographically isolated. Is experiencing barriers related to race, culture, ethnicity, or disability. Has been recently evicted from a residential setting, is residing within a shelter, or is at serious risk of being evicted from their place of residence. Has frequent involvement with the police, legal system, or frequently uses crisis/emergency services. Possible Exclusionary Criteria: All possible referral options in regards to available specialized development disability and/or dual diagnosis service providers have not been exhausted and/or recommendations from specialized service providers/consultation services have not been adequately followed through on. Demonstrates risk for unpredictable violence or assaultive behaviours.
