Program Description

Case managers will closely work with individuals with addiction issues who require support to live and work in the community. Case managers provide on-going support to individuals in order for them to stabilize, improve the quality of his or her life and to achieve goals. Case managers coordinate the services that individuals require to live in the community (i.e. mental health services, criminal justice, appropriate housing and other services).

Access Information

Referrals are accepted from professionals or self. To be eligible for the program, potential service recipients must meet one of the following criteria: currently using opioids or history of opioid use, interest in harm reduction for opioid use, interest in methadone or suboxone, presently taking methadone or suboxone and want to transfer to a holistic program, or presently taking methadone or suboxone and want help with tapering off.
