Program Description

The Mental Health and Justice Release from Custody Planning Program provides assistance to individuals currently in custody to devise release plans and case management follow up post-release. Services are voluntary and client-directed.

Access Information

Collect calls are accepted from correctional facilities. Individuals may self-refer, or referrals are accepted from any social service, hospital, Duty Council, the Private Bar, or criminal justice system staff. A referral form is preferred, but not necessary. Appointments are booked upon referral and there is no waiting list. The client must be aware that the referral has been made.
