Case Management/Supportive Counselling & Services - Addictions Supportive Housing

Program Description

The program is designed for people who have trouble maintaining housing due to their substance use and are looking at making positive changes in their lives. Participants will be working with an intensive addiction case manager onsite to develop an individualized approach to treatment. Housing within the ASH program may run for up to 364 days in length. Intensive case managers will be working with individuals to support the skills necessary to obtain and maintain stable and appropriate housing. This program is also available in Middlesex, Elgin and Oxford Counties.

Homeless Clients

Self-referrals and professional referrals. Referral form can be faxed or e-mailed. Contact intake for availability and date of next information session. Units are non-smoking. Must be homeless or at risk of homelessness due to substance use issues. Client must be motivated for change.
