Program Description

This case management program assists individuals facing significant barriers to recovery dealing with both addiction(s) and mental health issues. Clients are supported with their goals in areas such as addiction recovery skills, life skills, housing, employment and symptom management.

Access Information

Referrals processed through one-Link. In order for individuals to be appropriate for referral, they must meet all of the following criteria: not be in crisis; be a resident of Halton Region; have a mental health diagnosis or symptoms of mental illness; have a substance use disorder diagnosis or symptoms of substance misuse; not currently have a case manager within the addictions or mental health field; be voluntary; and can be safely supported in the community. Also, individuals must meet at least one of the following criteria: has visited an emergency department within the past year for substance use or mental health concerns; has been admitted to a hospital or residential addiction treatment program within the past year; has been admitted to a withdrawal management service within the past 30 days; or has experienced negative consequences related to their substance use in the following areas of their lives (must have three or more): physical health, relationships, legal, financial, employment. Referrals accepted from all sources. Participation is voluntary. If this is not a self-referral, please discuss this request for service with the individual before submitting referral. In order to facilitate the intake process, please include any available clinical reports, records, assessments and summaries.
