Program Description

For older adults 65 and up who are in mental health and/or addictions crisis (including dementia).

Offers: mobile crisis intervention and counselling to older adults; short-term intensive case management; harm reduction and concurrent disorders service; limited primary care and nursing; mental health and addictions assessment, counselling and referrals.

The initial visit will usually happen between 24 – 72 hours after making the referrals, based on a standardized assessment.
