Program Description

This program coordinates access to mental health services for adult clients with concurrent disorder, defined as co-occurring addiction and mental health issues. The case manager works with clients, mental health workers and other organizations to navigate the addictions and mental health system, and provides a link between service system, support system and care plans.

Clients may be referred by outside organizations or may self-refer. Clients must not be intoxicated. Please call for appointment.
